Three steps to record your request properly.

Open the Word file, fill in your details, the exact site address, your contractor's name and address, the approximate end-of-works date and the reception date you wish to set (typically 8 to 15 days after sending). If relevant, add the list of points to be examined jointly on the day. Print two copies.
Send by registered mail with acknowledgement of receipt
It is the only legally enforceable way. Carefully keep the bpost receipt and the signed acknowledgement of receipt. Without registered mail, you have no proof of dispatch in case of dispute over deadlines. Cost: ~7 €. For purchases under the Breyne Law, attach a copy of your notarial deed.
Follow-up at 15 days and procedure in case of silence
If the contractor does not respond within 15 working days, you can proceed to a unilateral reception in the presence of a sworn expert.
